Do I have to have had work experience in a hospital?

No, although it looks good and is valuable (and so if you can thats great) medical schools understand that it can be very difficult to actually get work experience in hospitals. It would look equally if not more impressive if your had volunteering experience (of any type but the more medically related the better) as that shows long term commitment to something.
